flourishboston on "contact form issues"

Dear Jetpack,

I am having issues w/ the contact form. I am trying to insert it into a post and it will not give me all of the posting options.

This is the only code available, regardless of which sections I'm trying to use:
[contact-form][contact-field label='Name' type='name' required='1'/][/contact-form]

I also need to have an email line and an optional comment line. I have updated recently, and everything else on my WP site is updated. I have also tried pasting this on another new post to see if it is any different.

Any thoughts on this?
